• 1. Browse the website: www.nhs.uk/the-nhs-app
• 2. Click on either download on the App Store or Get it on Google Play
• 3. Install the app on your mobile phone or tablet
• 4. Tap it to open it
• 5. Tap continue with NHS login
• 6. If you are under 13 years old then you are too young to use the app
• 7. If you are aged 13 to 15 years old tap “What to do if you’re aged 13 to 15”
• 8. If you are 16 years or older click continue
• 9. Tap accept all cookies
• 10. Enter your email address
• 11. Tap continue
• 12. Scroll to the bottom
• 13. Tap continue
• 14. Create a password and confirm it
• 15. Tap continue
• 16. Check your email account for the verification email
• 17. Click confirm email on the email you have received
• 18. Tap I have confirmed my email back on the phone/tablet app
• 19. Enter your mobile number
• 20. Your mobile will receive a text message with a 6 digit security code that will need to be entered in the app
• 21. Tap yes if you know your NHS number or no if you do not know It
• 22. If you do not know it enter your name
• 23. Type in your date of birth
• 24. Enter the post code SE21 8AU (Paxton Green)
• 25. Tap submit
• 26. Alternatively enter your NHS number
• 27. Tap continue
• 28. Accept conditions of use via the tick box and tap continue
• 29. Choose whether to take part in NHS App user research and tap continue
• 30. Scroll down to “prove your identity to get full access” and tap continue
• 31. Tap continue
• 32. “How to prove who you are without sending a photo of your I.D.”
• 33. Choose yes – I use online services and tap continue
• 34. Choose yes I have all 3 details and tap continue
• 35. Enter these details you would have received from Paxton Green Group Practice Reception Team.
• 36. Tap continue
• 37. Enter your name
• 38. Tap Continue
• 39. Enter your date of birth and tap “Submit details”
• 40. Tap continue with NHS Login
• 41. Enter you email address and tap continue
• 42. Enter your password and tap continue
• 43. Tap your 6 digit code which was sent to your mobile phone
• 44. Scroll down and tap continue
• 45. Tap “View your GP health record”
• 46. Tap continue
